Summary:

Retrograde ejaculation [RE] means that the semen is passed back into the bladder rather than ejaculated out through the urethra [the tube that passes through the penis that normally carries semen and urine out of the body]. The man experiences the sensations of orgasm [associated with contractions of the muscles at the base of the pelvis] without the sensation of semen passing through the urethra. RE is not common and is usually associated with a surgical procedure in the treatment of tumours [cancer] in the pelvis, bowel or genitals, and the use of some medicines, and spinal cord njury.
The diagnosis is made on the history of absence of ejaculation and can be confirmed by the presence of sperm in the urine following sexual intercourse – the urine looks milky. Libido and erection are usually unaffected, but may be impaired by the psychological effect (e.g. concern about sexual function following prostatectomy).
Fertility may be problem and this would require specialized management. There may be associated erectile dysfunction e.g. with spinal cord injury or diabetes [1].
Treatment depends on the cause, and an understanding of the effects of surgery or illness. Medication can be helpful, so it is important to discuss this with your doctor.
