Definition:
Persistent or recurrent ejaculation that occurs with minimal stimulation before, during, or immediately after penetration and before the man wishes it, over which he has little or no voluntary control, which causes him and/or his partner bother or distress.
This may be affected by age, novelty of the sexual partner and situation, and recent frequency of sexual activity. Duration of sexual intercourse less than 2 minutes suggests PE, but may range from 1-7 minutes [2]. Many men will experience rapid ejaculation on some occasions in their sexual experience.

There are three key criteria to PE:
  1. limited duration of intercourse
  2. loss or absence of voluntary control
  3. marked distress, bother, or upset between partners
PE does not usually occur with masturbation but is specific to intercourse.

PE may be
  1. lifelong: absence of voluntary control from the beginning of sexual activity.
  2. secondary: ejaculatory control is lost after a period of effective control.
  3. generalized: occurs under all circumstances since the problem started.
  4. situational: occurs with one partner and not another, or only in special circumstances or during a specific sexual activity.

PE cannot be assessed solely on the man’s female partner’s inability to reach an orgasm or the time it takes her, because of the difference of response and interactions between men and women [See: Differences in Male and Female Responses]. Some women require intense or prolonged stimulation prior to penile entry which they may not receive, and many women are unable to climax with penile thrusting alone, and require clitoral stimulation before, during or after intercourse - which they may not be getting.

About 30% of men with PE also report having problems achieving and/or maintaining an erection [3].
